Hello friends, today we are going to discuss an important but often ignored topic: spinal cord injury. This topic is of immense interest to a large group of people across the world who are living with spinal cord injuries. These injuries not only cause distressing disability but also affect the quality of life of those affected and their families. Spinal cord injuries are on the rise due to high-velocity road traffic accidents, occupational hazards, falls, and general trauma.

The spinal cord is the connection between the brain and the lower body, and any injury to it can cause several problems in men, including defecation or passing stool, bladder problems, penis problems (such as erectile dysfunction, ejaculation problems), fertility problems, and lower limb loss.

After spinal cord injury, the first step for patients is to save their life, followed by regaining as much function as possible. However, there is usually residual loss of function in one or several organ systems. Rehabilitation for the lower limb, fecal part, and urinary part is easily available to patients, but erectile dysfunction, ejaculation problems, and fertility issues are distressing morbidities that spinal cord injury patients face. Sexual problems after spinal cord injury are complicated, and patients may suffer from a wide range of problems that depend on the mode and extent of the injury.

To handle these problems, it is recommended to break down sexual dysfunction into the three main components: erections, ejaculation, and semen. Once we have a diagnosis, we can offer a customized solution to the patient.

Several treatment modalities are available for each of these components, including medication, therapies, penile rehabilitation, shockwave therapy, PRP, electrical stimulation, Botox, self-ejaculation, electroejaculation, vibratory massage therapy, and penile implant.

Patients may also benefit from seminal vesicle aspiration, IUI, and TESA for IVF.
